Creating & Duplicating a Smart Forms

This article provides instructions on how to create a new smart form and how to duplicate a smart form.

Creating a Smart Form

Navigate to the Forms tab from the Library tab within your menu and click into the Smart Forms section. Then follow the instructions below to create a new smart form:

  1. Click + Create Form at the top right of the Forms page.

  2. Enter a Title (click the pencil icon) and a Description (supports bold, italics, underline, and code).

  3. Build your form using the following components: Add Questions, Add Text, Add Signature

  4. Save your form using the Save button in the top right

    1. Saving a Smart Form will not push changes to existing cases. In order for existing cases to receive updates to a Smart Form, it must be published (See Smart Form Versioning for additional information)

🔹 Add Questions

Click + Add Question to insert a question block.

Customize:

  • Question text

  • Instructions (optional)

  • Required checkbox

  • Type (see below)

  • Image upload (.jpg, .jpeg, .png only)

Supported Question Types:

Text

Short answers

Paragraph

Long answers; Open-ended responses

Number

Numeric only

Dropdown

One selection option

Radio

One selectable radio button

Select

Multiple checkboxes

Date

mm/dd/yyyy format with calendar picker

💡 Tip: Reorder options using drag handles and delete with the trash icon.

Conditional Questions

Make questions appear only when specific conditions are met.

  1. In any question, check Only appears when.

  2. Set conditions based on answers to earlier questions.

  3. Use and/or logic for multiple conditions.

💡 Tip: Conditional questions can be required—something not possible in legacy forms.

🔹 Add Text Components

Useful for grouping questions or providing instructions. You can include:

  • Formatted text (bold, italic, underline, code)

  • Images

🔹 Add Signature Component

Used to collect digital consent. Only one signature field is allowed per form. It can be set as required.

Duplicating a Smart Form

Once changes to a Smart Form have been saved, an entire form can be easily duplicated.

  1. Click the ellipsis menu (•••) in the top right corner of the Smart Form builder

  2. Click Duplicate to duplicate and rename the entire form

  3. Click Duplicate again to confirm the Smart Form duplication
