Instructions for resetting your password and logging into Force.

**If you have never logged into Force you should follow the instructions in your Welcome email to create your password.

Or you may have also received a Welcome text message (Patients only), it requires a “Y” reply to opt into text messages. You may then use your phone number to login and set a password.

To reset your password please walk through these instructions:

If your email or phone number (only patients may use phone numbers) is not automatically populating the ‘Email or 10 digit Phone Number Field’ please enter that first and click Continue.

If you have forgotten your password click the prompt "here" for “Forgot your Password?”

Next, enter your email or phone number (only patients may use phone numbers) and select ‘Send create password link.’

Next you will see a message instructing you to check your email or phone for a reset password link.

You will receive an email with the subject line “Set your Force password,” or a text message with a link. The email will have a button that says ‘Set Your Password.’

Next you will see a page that auto-populates your email or phone number and asks you to enter your date of birth. This is to verify your identity.

Create a password following the criteria given and confirm it by entering it a second time. Use the eye icon to view what you typed if necessary.

Select Log In.

Your password is now created and you will be logged into Force.
