Completing Your Forms

This article guides you through each step of completing your forms on Force

Forms help your care team collect important information before and after your procedure. Here’s what to expect and how to complete them successfully.

Confirming Your Procedure Details

Before you begin, you’ll be asked to confirm your procedure information.

  1. From your Task List, click “Complete your forms”

  2. A confirmation screen will appear showing your:

    • Procedure name

    • Laterality (right, left, or both sides)

    • Surgery date

  3. Check the confirmation box and click “Submit” to continue.

If you try to click Submit without checking the box, you’ll see an error message: “You must confirm your surgery to continue.”

Note: You’ll only need to confirm this once per session

Not Correct? Let Us Know

If something doesn’t look right — such as your surgery date or provider name — and your surgery hasn’t happened yet, you can request a correction:

  1. Click the “Let us know” button below the procedure details

  2. You’ll be asked to review these details:

    • Surgeon

    • Procedure

    • Date of surgery

    • Location

  3. Check any information that’s incorrect

  4. Add any comments in the text box provided

  5. Click Submit

You’ll then see a thank-you message. Your request goes directly to your care team — no changes will be made until your care team updates your record.

Note: This option is only available before your surgery. After your surgery date, the link will no longer appear.

Completing Your Forms

After confirming your procedure, you’ll begin your first Form. Here's how the experience works:

  • You’ll see the form name at the top of the screen

  • If your surgery involves both sides (bilateral), the laterality (Right/Left) will be included in the form title

  • Just below, you’ll see a short description that stays visible as you answer each question

  • Forms are one question at a time — just read and respond!

As you move through the form:

  • A progress bar will show how far you’ve gone

  • In some cases, your answer to a question may trigger a follow-up question. Additional questions will only appear based on your response to a prior question.

  • Click Previous at any time to go back and review or change your answers

When you finish the last question:

  1. Click Submit

  2. You’ll see a “Thanks for submitting” confirmation

  3. If more forms are available, click to begin the next one
