Once a Smart Form has been saved, you can assign scoring to the form. This is a net new feature of Smart Forms compared to our legacy Outcome Forms.
How to Score Smart Forms?
Click Assign Scoring at the bottom of the Smart Form builder.
Only dropdown, radio, and select questions can be scored.
Enter integer values for each response.
Min and Max scores are automatically calculated.
Use Clear, Cancel, or Save as needed
Clear: will reset score values to "0"
Cancel: will bring you back to the Smart Form builder
Save: will save your current scoring configuration
