What Are Smart Forms?

Where to Find Smart Forms
Go to the Library tab in the left-hand navigation bar.
Click on Forms.
Here you’ll see a list of all Smart Forms for your organization with details like:
Name
Creation date
Assigned protocols
Last published version
🔒 Template Forms
Forms with a lock icon are Force-managed templates (e.g., validated PROMs). These cannot be edited by organizations.
How to Preview a Smart Form
Click the "Preview" button in the Smart Form builder header.
By default, the preview will display the patient view of a Smart Form showing each question individually with a progress bar.
To change the preview to the provider view of a Smart Form, click the dropdown in the upper left corner.
The provider view will display all questions in a traditional form layout.
